What services do you offer?

Capital Health & Wellness provides integrated care across Internal Medicine, Primary Care, Infectious Disease, Advanced Wound Care, and Medical Aesthetics—all within one coordinated practice.

How do I schedule an appointment?

 

Appointments can be scheduled by phone at (281) 941-5740 or through our CHW Patient Portal. Our team will help determine the appropriate provider based on your needs.

Do I need a referral to be seen?

Most services do not require a referral. Certain insurance plans may have specific requirements, and our team is happy to help verify this for you.

What can I expect during my first visit?

Your first appointment is focused on understanding you and your health needs. You can expect time to discuss your medical history and current concerns, review of medications and past treatments, a physical evaluation (when appropriate), discussion of preventive care and screenings, and a clear explanation of next steps and treatment options. Our providers take time to listen and answer questions.

Can I receive multiple services within the same practice?

Yes. Many patients receive care across more than one specialty. Our integrated model allows providers to coordinate treatment plans seamlessly.

What does “integrated care” mean?

Integrated care means your providers work together within one practice—sharing records, collaborating on treatment plans, and coordinating next steps so your care feels connected and organized.

How does integrated care benefit me?

Because Capital Health & Wellness offers multiple services under one practice, your care may involve collaboration between providers. This means your medical information is shared securely within our team, providers communicate directly, treatment plans are coordinated (not fragmented), and you won’t need to repeat your history at every visit. Integrated care helps ensure your health is viewed as a whole.

Do you accept insurance?

We accept most major insurance plans. Our staff can help confirm coverage prior to your visit.

Will my doctors communicate with each other?

Yes. Our physicians and specialists collaborate regularly, allowing your care to remain aligned across services.

Who manages my overall care plan?

Primary Care and Internal Medicine often serve as the central point of coordination, guiding your care across specialties when needed.

How do I access test results or communicate with my provider?

Patients can access results and communicate through our patient portal or by contacting our office directly.
